| | | | | | | | | Background: | On 11/14/22, the Board Approved and authorized the Chairman to sign the COPCN to Century Ambulance Service, Inc. with the expiration date of December 31, 2024.
On 7/10/23, Century Ambulance advised NCFR that they will be purchased by RG Ambulance Service, Inc., and requested a new COPCN reflecting the name change.
On 8/16/2023 the Board Approved and authorized the Chairman to sign the COPCN to RG Ambulance Service, Inc., dba Century Ambulance Inc. as requested.
On 9/12/23, Century Ambulance advised NCFR that the name should read RG Ambulance Service, Inc. DBA Century Ambulance Service, and requested a new COPCN be issued. On 10/31/23 NCFR received an initial application and supporting documents from RG Ambulance Service, Inc. DBA Century Ambulance Service to complete the request to issue a COPCN to RG Ambulance Service, Inc. DBA Century Ambulance Service.
