The initial agreement allows for a total of three (3) extensions in one (1) year increments. Henry Schein is renewing the contract with the exception of the Pharmaceuticals Group, line items 216 through 249 and Fluids Group, line items 97 through 102. Staff requests the Board approve Amendment Number 3 - Third Extension to the Contract with Henry Schein, Inc. (CM2046-A3) for medical supplies for the period beginning March 24, 2017 and ending March 23, 2018.
