The initial agreement allows for a total of three (3) extensions in one (1) year increments. QuadMed is renewing the contract with four (4) amendments to Line Item Pricing. Staff requests the Board approve Amendment Number 3 - Third Extension to the Contract with QuadMed, Inc. (CM2044-A3) for medical supplies for the period beginning March 24, 2017 and ending March 23, 2018.
