Item Title:The Standard Group Insurance Amendment

Background:

On September 21, 2016 the Board approved the Chairman to sign the "Request for Group Insurance Amendment", Contract No CM1497-A3 with the Standard for Life and Long-Term Disability Insurance. This amendment adjusted the rates for all lines coverage in accordance with the proposal submitted during the bid process as well as outlined guarantee issue limitations for the last open enrollment period.

It was recently realized that the above amendment neglected to amend the renewal date to January 1, 2020 and change the date for which our premiums would be adjusted due to age changes to January 1st of each year in conjunction with our new enrollment period. 

Request:

Approve and authorize the Chairman to sign the Request for Group Insurance Amendment with the Standard Group, Contract No. CM1497-A5, for Life and Long-Term Disability Insurance.
