Background:

On September 23rd 2019 the BOCC approved changing the Vision insurance carrier from Florida Blue to EyeMed per the Insurance Committees recommendation. EyeMed offered a flat renewal of our current vision rates and a 4 year rate guarantee. 

 

HR verified that the EyeMed application is also considered a contract so a contract number has been assigned.

Approve and authorize the Chairman to sign the EyeMed Application for Vision Care Benefits, Contract No. CM2764.
