Item Title:Health/Dental/Vision/Life Benefits for 2023

Background:

Nassau County currently offers four (4) medical plans through Aetna; ACO2500 as the base plan and two (2) buy up plans; HNO1000 and OAMC1500 (PPO).  The fourth (4) plan HDHP3000 is a high deductible health care plan (HDHP) with a health savings account (HSA), which is also considered a base plan.  The County currently contributes $1,500 into the employees HSA account for since coverage and $2,250 for dependent coverage.  This is currently contributed biannually; 50% in January and 50% in July.  Nassau County currently offers Dental DHMO and PPO plans through Humana Dental.  Nassau County currently offers Vision through EyeMed.  Nassau County currently offers Life and Long Term Disability (LTD) through the Standard.

 

With no plan changes, following negotiations with the County, Aetna is offering a flat renewal for January 1, 2023 plan year. Humana is in a rate guarantee for DHMO and PPO Dental until January 1, 2024 . EyeMed is in a rate guarantee for Vision until January 1, 2024.  The Standard is in a rate guarantee for Life and LTD until January 1, 2024.

 

The Bailey Group and the Insurance Committee met on March 2, 2022 and March 25, 2022 to help come up with a plan to try and minimize health care costs and help the employees with the costs associated with the plans. The Insurance Committee recommends the following; accept the flat renewal from Aetna with the plans staying the same and the contribution rates from the County staying the same, but also offering a new HRA account for all active, non-union, employees on the ACO2500, HNO1000, and OAMC1500 plans. The County would contribute a one-time amount of $500 into each account for 2023.  Retirees and HDHP3000 plan are excluded from this.  Unions representing County employees (L630 and L3101) could bargain to be included in this benefit.  Aetna will offer to our retirees that choose the ACO2500 plan, but live out of area, a HMO network in their location if one is available at no price difference to help with our retirees that live out of the plan area.

 

In addition, it is recommended to continue the existing agreements with Humana Dental, EyeMed Vision, and The Standard Life and LTD which are all in a rate hold until January 01, 2024.

 

The Insurance Committee voted and recommends that the BOCC approves renewing the Health, Dental, Vision, Life/LTD plans as follows;

  1. Approve the flat renewal from Aetna with adding the HRA component and out of area retiree plan option.
  2. Approve continuing the existing agreement with Humana for DHMO and PPO Dental insurance with a rate hold until 2024.
  3. Approve continuing the existing agreement with EyeMed for vision insurance with a rate hold until 2024.
  4. Approve continuing the existing agreement with The Standard Life /LTD insurance with a rate hold until 2024.

Human Resources requests approval from the BOCC to renew the Health, Dental, Vision, and Life/LTD, as stated above.

 

Request:

Consider the following: 1) Approve the flat renewal from Aetna, adding the HRA component and out of area retiree plan option; 2) Approve continuing the existing agreements with Humana for DHMO and PPO Dental insurance, EyeMed for vision insurance, and The Standard Life /LTD insurance, all with a rate holds until 2024.

Financial/Economic Impact to Future Years Budgeting Process or Effect on Citizens:
Insurance rates are included and approved in departmental budgets each year.  Maintaining health, vision, and dental benefits with flat rates are crucial for employees and work toward employee retention and recruitment.
Action Requested and Recommendation:
