RESOLUTION(S)
A.Approve a Resolution of the Board of County Commissioners of Nassau County, Florida, proclaiming the month of October 2019 as Manufacturing Month.

B.Approve and authorize the Chairman to sign a Memorandum of Understanding with the Nassau County Health Department for the County Medically Indigent Program (CMIP). (CM2723)
C.Approve and authorize the Chairman to sign the contract between the Board of County Commissioners and State of Florida Department of Health for operation of the Nassau County Health Department for FY 2019/2020. (Contract CM2724)
D.Approve Finance Package 2019-29.

E.Continue discussion regarding the Stewardship District and House Bill 1075.
F.Discussion on the status of paving, design and right-of-way acquisition of Crawford Road.

G.Approve and authorize the Chairman to sign a corrected Memorandum of Understanding with JEA for Design and Construction of the water main for the Pages Dairy Project from Felmor Road to Chester Road, Contract No. CM2716.
H.Consider the following: 1) approve the Evaluations Committee's Technical Evaluation for Request for Proposals received in response to Bid No. NC19-006, Fire Station 71 Design/Build; 2) award bid to the top ranked firm, Core Construction Company of Jacksonville, Inc.; and 3) authorize the Chairman to sign the Design Build Agreement, Contract No. CM2729.
I.Approve the Evaluation Committee's final scoring and ranking of proposals received for Professional Engineering Services for the County Road 115 Improvements Project; and authorize staff to begin negotiations with the top ranked firm, CDM Smith and to bring back a contract for Board approval.
J.Award Bid No. NC19-015 to Anderson Columbia, in the amount of $6,769,323.25, for the County Road 115 Improvements Project; and authorize the Chairman to sign Notice of Award and Contract No. CM2731. Funding Source: 63470541-563100 115SB.
K.Approve and authorize the Chairman to sign Work Authorization CM2685-WA03 for S2l, Inc., in the amount of $188,485.10, for Engineering Services Operation, Monitoring, Maintenance, and Reporting for the Landfill Gas Collection and Control System at the West Nassau Landfill. 01361534-531325 and 01362534-531325.
L.Approve and authorize the Chairman to sign Work Authorization No. CM2685-WA06 to S2L, Inc., in the amount of $56,071.62, to provide sampling, laboratory analyses, and reporting services for the West Nassau and Lofton Creek Landfills.  Funding Source(s): 01361534-531000; 01362534-531000; and 01363534-531000.
M.Approve and authorize the Chairman to sign a Budget Transfer for purchase a vacuum truck. Funding Source: From 03402541-546510 DRAR (Drainage Maintenance) to 03404541-564002 (Equipment Greater Than $50,000).
N.Approve a Resolution adopting the Five-Year Capital Improvement Plan (CIP) in compliance with adopted Financial Policies, Section V, Capital Improvement Policy 5.6 of Resolution 2016-131, as amended, and pursuant to the Impact Fee Ordinance, Ordinance 87-17, as amended, extending for three years the date to refund impact fees shown as a funding source for projects in the first three years of the CIP; providing for an effective date.
PUBLIC HEARINGS – The following matters under consideration are legislative matters, and the Non-Quasi Judicial Procedures will be utilized for the following: County Attorney to read the procedures.

Consider LDC19-008, an Ordinance of the Board of County Commissioners of Nassau County, Florida, amending Article 28, Supplementary Regulations, specifically amending Section 28.02, Erection of More than One Residential Dwelling Unit on a Lot or Parcel; providing an effective date.

P.Public Hearing to consider adoption of the FY2019-2020 millage rates and budget.
PUBLIC HEARINGS – The following matters under consideration are Quasi Judicial matters, and the Quasi Judicial Procedures will be utilized for the following: County Attorney to read the procedures.

Consider FD19-003, the Final Development Plan for Phase One Orange Avenue Path, a 10’-wide multi-use trail extending from the Enclave at Summer Beach to Amelia Island Parkway within the Summer Beach PUD.
